People with long-term conditions are the most frequent users of healthcare services. In England they use 50 percent of all GP appointments and 70 percent of all inpatient bed days.
More than 60,000 people – over 25 percent of the population – in the Barnsley region live with chronic illness. NHS Barnsley represents a significant commitment to help people deal with the problems that living with a long-term condition can cause. Bosch Healthcare provides technology and decision support expertise to the general practitioners as well as clinical and social teams within the NHS Barnsley. Patients in the programme received an easy-to-use Bosch home appliance to send the vital signs, and to answer a series of questions about their health and symptoms. This data will be transmitted to the care managers at Barnsley.
- 2011 NHS Barnsley provides the Bosch Telehealth Plus System for chronically ill patients with initial 150 high intensity patients living with congestive heart failure (CHF)
- In a second phase a further few hundred patients with long-term conditions such as chronic obstructive pulmonary disease (COPD) and Diabetes will be chosen to take part in the programme with the aim of rolling it out to thousands of patients in the third phase
- A key objective of NHS Barnsley aims to provide better care and quality of life for patients in their own home and help to avoid unnecessary costs
